Physical Therapist preferrably with home-care experience with a minimum bachelors in PT preferrable Masters in PT who is both intuitive and has a strong sense of team.
Plans and administers medically prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts. Maintains confidentiality. - Provide diagnostic and therapy services with appropriate professional judgment for skilled care. -Participate in the identification and referral of patients through screening, informal evaluations and communication with other persons. -Re-evaluate skilled needs, modifies treatment plans, and prepares discharge summaries as warranted. -Prepare and maintains written and electronic documentation as required for the profession and the department in a timely manner, including billing, evaluation reports, treatment plans and daily progress notes.
